Here you will find information on requesting a transcript, NCAA transcript requests (student NCAA number required), Dual Enrollment and college application information. Please note that all students requesting a transcript will be required to pay a fee.
Transcript Request Form
This is the form needed in order to have a transcript sent to a school if you attended Rise Hybrid Private Academy during K-8. Please note there is a $10 fee for all students who are not currently attending Rise Hybrid Private Academy and would like an official transcript. Please make sure to email your completed request to risehybridkids@gmail.com.
High School Transcript Request Form
This is the form needed in order to have a transcript sent to a school if you attended Rise Hybrid Private Academy during 9-12th grade. Please note there is a $20 fee for all students who are not currently attending Rise Hybrid Private Academy and would like an official transcript. Please make sure to email your completed request to risehybridkids@gmail.com.
Dual Enrollment at the Community College forms
These are the forms needed in order to have a transcript sent to a school if you plan to Dual Enroll at community college. First read through this link https://www.nvcc.edu/dual-enrollment/checklist.html to see an example of what is required to enroll in community college as a dual enrolled student. Once you have read this and discussed the process with your parents or guardians we will assign a staff member at Rise Hybrid Private Academy to help guide you through the process.
